Treasure Health Systems will help client to identify financial needs. We will work with the client to access entitlements and financial assistance and to follow up with applications and recertification date’s.

Treasure Health Systems trained staff shall document that the consumer has entitlements or document efforts to assist the consumer in obtaining entitlements for which he/she may be eligible. Examples of entitlement data include:

  • Copies of Supplemental Security Income Determination letters.
  • Dept. of Social Services award letters
  • Birth Certificate
  • Social Security Card
  • Medicaid Card
  • Immunization Record
  • Food stamps
  • Income Supplements
  • Transitional Emergency
  • Name, telephone number and address of all the client’s medical doctors (primary care physicians, dentists, etc.)
  • Legal Documents
  • Medical and Housing Assistance
  • TEMHA) program forms
  • MD Disability Determination Services letters.
  • Any existing IEP Plans, 504 plans and behavior modification plans (If applicable)
